Prepare for your first visit
Here is a check list of what to bring to your first visit to the doctor’s office:
- The filled new patient form (Online or PDF)
- Current health insurance card
- Valid state driver’s license or valid passport
- All diagnostic imaging reports including MRI, Ct-scans or X-rays
- A copy of your medical records from your primary doctor or previously treating Doctor. If you don’t have them, please ask your doctor to send them.
- A list of all of your current prescription medications and nutritional supplements you are taking.
